So far, Mabinogi has most impressed me with its combat.  Its not as fast paced as some and most games have more skills.  However, its the only one I've played where it is truly important to pay attention to what your enemy is doing and the only successful tactics are the ones that don't involve getting hit.  The game does technically have an auto attack, but using it will pretty much automatically get you killed.

While I don't think its the best, Granado Espada deserves honorable mentions for having a combat system distinctly different from the other games listed here.

I concur.  These are my 2 favorite combat systems in that order.

played: almost every mmo out there

Planetside when it was in its prime hands down. No game to date has given me that feeling of being part of an all out war like planetside did.

for me DAOC

Lots of positional styles, and follow up styles had to really know and watch what you where doing.  I personally enjoy a game you actually have to work at to play.  I hate to stand there and hit one or two buttons.

I have not played AOC or COH so can't comment on them :(

More like the worst.  With buffs a simple duel could be 3+ hours long.  On top of that only 4.5 combat professions had access to attacks or weapons which directly damaged the Mind attribute (swordsman, rifleman, pikeman when mindfire DoT pikes started turning up, TKA, and BH with eyeshot) and therefore rendered every single other combat profession sans combat medic completely worthless in PvP.

I don't know about the rest of the people around here 3 hours for a duel is not fun nor was having well over half of all the combat professions in the game as worthless in PvP as an Image Designer.  I know, I played Master Carbineer with Commando 4300.  The only thing I was good for was hucking thermal detonators at jedi as it was the only attack in the game that bypassed saber block.  Not that it mattered though because my accuracy was such shit that I could 15+ throws without hitting.  Or the fact that the splash damage on the grenade often times had a larger distance than the throwing distance (meaning that you toss a grenade, miss your target because of the horrendous accuracy, but hit yourself with the splash 100% of the time).

There were a lot of cool things about PreCU SWG.  Combat was not one of them.

That sounds like a really excellent combat system. I would play DDO if it had more pvp features

Planetside is in a league of it's own.

Thereafter I'm in agreement with the OP.

COH and D&D online have outstanding combat systems. Fast intuative and interdependant.

DDO, 2 of the games strongest points is the combat system & character creation.
